What you will need: Cricut Machine Dr Checkup Cartridge cardstock in your colours you want to use, I used White, cream & black. On Page 61 of the Dr Checkup booklet Cut out Icon in black, Icon Shift in white & icon layer in cream at 4inches. For the ER visit Cut out Phrase in cream, Phrase shift in white and Phrase layer in black all cut about 2 to 3inches. I had paper that had square tiles and cut some blue cardstock to fit in the middle. Piece all your cuts together & place on the layout.
